Every year, tens of thousands of people enrolled in private Medicare Advantage plans are denied necessary care that should be covered under the program, federal investigators concluded in a report published on Thursday. The investigators urged Medicare officials to strengthen oversight of these private insurance plans, which provide benefits to 28 million older Americans, and called for increased enforcement against plans with a pattern of inappropriate denials.

A cascade of care is a seemingly uncontrollable sequence of medical services that is driven by the desire to avoid even the smallest risk of a bad outcome. Cascades prompted by low-value medical tests or incidental findings (on any test) are both common and consequential, often having limited clinical value and potential for harm.

The HHS Office of Inspector General has released a favorable advisory opinion on physician ownership. The decision should be good news for some manufacturers of innovator medical devices—specifically, companies in which physician-inventors have received ownership interests in exchange for their product development contributions, but whose orders represent only a limited proportion of the companies’ sales—and the health systems, hospitals and ambulatory surgery centers who are the potential customers. |

Brain illness and injury, with a focus on dementia-related diagnoses, occupy a high volume of the 1,495 proposed changes to the ICD-10-CM diagnosis code set by CMS in the fiscal year 2023 inpatient prospective payment system proposed rule. At the same time, CMS also introduced a minimal amount of new ICD-10-PCS procedure codes, including codes for reporting knee joint replacements.
