If you haven’t already started, there are two steps you should take today: dual coding and tracking trends.

Dual coding. Your ASC coder needs to begin dual coding from not only a “hands-on experience” perspective but also from a documentation analysis perspective. Comparisons of ICD-10 diagnosis coding with current real time documentation per physician will identify specific documentation deficiencies and enable your ASC’s physicians to correct ambiguous practices now.

Tracking trends. ASC staff have long been advised to trend physician documentation deficiencies, and your ASC coders are in the best position for determining whether a physician’s current operative documentation can be reported to the highest specificity in ICD-10. Trending will provide an analysis of which physicians may need more assistance than others before and after October 1.
