Survey forms to participate in the fourth quarter of ASCA's 2012 Outcomes Monitoring Project were faxed to all ASCs this week.
As in the past, you can submit your survey responses online, by fax or by email. The deadline for you to submit your completed survey is February 15, 2013.

ASCA's Outcomes Monitoring Project is a member benefit open to all ASCA facility members. Participation is free to all ASCA facility members.
