Instructions

Please follow the instructions below when completing your 2012 ASC Employee Salary & Benefits Survey. If you have any questions, please visit our FAQs section or contact us.

Salaries (except Medical Director):
  • Please report salaries for full-time employees only.
  • List the January 1, 2012 salary for each position for which your ASC employs staff.
  • If you have no such position, please leave it blank.
  • If you have more than one person in the position, give the average salary of the individuals filling that position as of January 1, 2012.
  • You should report the annual salary. If you have an employee who is paid by the hour, please multiply the hourly rate by 2080 to calculate the annual salary. Use our salary calculator below!
  • Please do not include bonuses in the annual salaries.

Medical Director:
  • Do not include any fees the medical director receives for providing medical care to a patient.
  • If the medical director works part-time report the % of full-time work that the medical director is expected to work on ASC business. For example, if an anesthesiologist serves as your medical director but spends half of his/her time providing anesthesia services for which he/she or your ASC bills patients or insurers, you would report 50% of his/her salary as being for your medical director.
  • Please express time worked as a percentage of 100 (e.g., report 10% instead of .10).

Benefits:
  • Please answer these questions for full-time employees only.
  • If you do not employ any full-time employees, do not answer these questions.

ASC Statistics:
  • Please include part-time employees when calculating FTEs.
  • Please answer all of these questions even if you only have part-time employees.
